Montgomery County Mental Health Core Service Agency has contracted case management services to St. Lukes House, Inc. for individuals not otherwise eligible in the public mental health system. The case management services include, but are not limited to, providing linkages to community based services and resources, crisis intervention services, and facilitating consumer relationships with individuals and organizations in the community which the consumer finds supportive. The services may not be mental health related but are intended to support the individuals ability to function as independently as possible in the community.
Case management is time limited and is typically available to consumers for a period of up to nine months. Up to thirty seven consumers are served in this program.
The housing facilitation services funded through this program are intended to assist consumers who are tenants of Housing Unlimited Inc. and Hughes Neighborhood Housing with maintaining their independence. Regular house meetings focus on the opportunities available to residents to maximize their level of satisfaction with their living situation. The team collaborates with tenants to develop a system of conflict resolution which supports their ability to constructively resolve interpersonal and house related conflicts. Approximately 95 consumers are served in this program.
